Here you can download an application to fill out or schedule an interview with our team to apply for a jacket.​
Please only do one or the other. If you do both we will only consider your interview application.
Written Application
This is the written application. You will be asked to answer a few questions on what a letterman jacket means to you, your achievements as a varsity participant, as well as asked to provide a mentor recommendation. ​​
Applications are due October 31st, 2025
Interview Option
Here you can schedule an interview with a team member instead of submitting a written application, the questions will be the same as on the written application. Please submit a mentor recommendation along with scheduling the interview.
Be sure to schedule before October 31st 2025

Nominate a Student
As a coach, teacher, or mentor, you see your students work hard and overcome challenge. If you have a student in mind that you believe is deserving of a letterman jacket to represent their achievements, please nominate them for an award.
There is a short form available that will take no more than fifteen minutes to fill out.
